Seven independent studies have confirmed that AI agents fail 70–95% of the time, according to a new book by VectorCertain LLC founder and CEO Joseph P. Conroy. MIT's NANDA study reported that 95% of enterprise AI pilots deliver zero measurable financial return, based on 52 organizational interviews and 153 senior leader surveys. The urgency of the book's message was underscored in January and February 2026 by a cascade of AI agent security failures.
